Booking and Deposits

Booking Deposit and Payment Policy

A $100 booking deposit is required at the time of scheduling to reserve provider time. Full payment is required prior to services rendered.

Deposit Rules

A valid card on file is required for all appointments, programs, and memberships
Full payment is required prior to services rendered
Booking deposits cannot be transferred to future appointments

Cancellation 24+ Hours in Advance

The $100 booking deposit is fully refunded.

Cancellation Less Than 24 Hours

The $100 booking deposit is forfeited and cannot be refunded or transferred.


Deposit Application

How the $100 Booking Deposit Is Applied

Aesthetic Procedures and Injectable Treatments

All aesthetic services require a current Good Faith Exam (GFE) on file.

If a GFE has been completed within the last 12 months, the full $100 is applied to your appointment
If no current GFE is on file: $30 is applied toward the GFE and $70 is applied toward the treatment

Membership Consultations

All memberships require a consultation. Consultation fee: $100.

If you enroll in a membership within 7 days of your appointment: $50 is applied to your first month
$50 is retained as the consultation and provider time fee

Discovery Intake

Discovery Intake Policy

The Discovery Intake is a required medical evaluation prior to enrolling in hormone, weight loss, or peptide programs. The fee is $395 due at booking.

Credit Application

If you enroll in the following program within 7 days of your Discovery Intake, the $395 will be applied as a credit toward your program.

Metabolic Reset Program (Hormones + GLP-1)

Important Limitations

If enrollment does not occur within 7 days, the $395 cannot be applied as a credit. This credit does not apply to standard monthly hormone memberships.

Programs and Packages

All packages require a signed agreement, card on file, and auto-pay authorization
Package fees are non-refundable once care has begun
Missed appointments, delayed follow-ups, or non-compliance do not pause or extend program timelines

Monthly Memberships

Auto-pay and card on file required at all times
3-month minimum commitment required
After the initial term, memberships may be canceled with 7 days written notice
Membership pricing does not include pharmacy costs unless otherwise stated

Card on File Requirement

A valid card on file is required at all times for all active patients, programs, and memberships.

It is your responsibility to update your payment information if your card expires, is lost, or is replaced
Failure to maintain a valid payment method may result in delayed or paused care

Late Fees

Payments not received within 3 calendar days of the due date will incur a $25 late fee.

Non-Payment Enforcement

Accounts 7 days past due may result in the following until the account is brought current and a new valid card is on file:

Unenrollment from membership programs
Paused care
No prescription refills until account is current

Medications and Pharmacy

Medications and Pharmacy Services

All prescriptions require prepayment prior to pharmacy submission
Medications ship directly to the patient
Pharmacy fees are non-refundable once submitted to the pharmacy

Cancellation and No-Show

Cancellation and No-Show Policy

Appointment times are reserved exclusively for you. We ask that you provide adequate notice if you need to cancel or reschedule so we can offer that time to another patient.

Cancellations

At least 24 hours notice is required to cancel or reschedule
Cancellations within 24 hours result in forfeiture of the $100 booking deposit
Appointments canceled within 24 hours or missed without notice will be charged in full

No-Show Appointments

Forfeiture of the $100 booking deposit
Appointment charged in full
Future appointments after a no-show may require full prepayment
